Participant Agreement, Release and Assumption of Risk
Prior to beginning your activity with the San Juan Outdoor School Adventures, LLC, you will be required to sign a "Participant Agreement, Release and Assumption of Risk" form. A parent or legal guardian is required to sign the form for any child under the age of 18. If you will be bringing a child who is not your own, please have the parent of the child read and sign a copy of the form in advance. You must submit an original copy of the signed form in order for a minor to participate in any activity with the San Juan Schools. We ask that you read and consider the terms and conditions of our form carefully.
